Smoky Mountain Home Health and Hospice has achieved “We Honor Veterans” Partner Level Two. Hospice-Veteran Partnerships (HVPs) are coalitions of community hospices, Department of Veterans Affairs (VA) facilities, end-of-life care and community organizations, and others working together to ensure excellent care of veterans and support for their families is available at the end of life for our nation’s veterans.
The national HVP program is a joint effort of the VA Hospice and Palliative Care Program and the National Hospice and Palliative Care Organization. We Honor Veterans (WHV) provides resources to promote veteran-centric educational activities, increase organizational capacity to serve veterans, support development of strategic partnerships, and increase access and improve quality.
Of 2.4 million deaths in the US each year, approximately 680,000 are veterans. By becoming a “We Honor Veterans” partner, hospices such as ours will become better prepared to provide veterans with access to quality care.
